Item 5 A’s Intervention to Increase Smoking Cessation in Primary Care(2019-04-12) Mobolade, ChristianaTobacco use remains the leading preventable cause of death in US adults. Healthcare providers have frequent contacts with their patients yearly in primary care clinics and are well positioned to identify patients who smoke and to assist with smoking cessation. The Agency for Healthcare Research recommends a comprehensive smoking cessation intervention, the “5 A’s,”; to ask, advise, assess, assist, and arrange smoking cessation interventions for patients who smoke (Grandes, Cortada, & Arrazola, 2018). This evidence-based project shows the effectiveness of the 5 A’s comprehensive smoking cessation intervention in every day routine consultation in primary care to increase smoking cessation.Item Behavioral Approach to Improve Insomnia in Older Adults(2019-04-12) Chacon, AlidaInsomnia is a sleep disorder highly prevalent, especially among older adults, and it is associated with cognitive impairment, increased risk for falls, depression, decreased quality of life, and increased healthcare utilization. Good quality sleep is essential for good health and quality of life. There is ample evidence that supports the use of behavioral interventions as a primary treatment for insomnia. This evidence-based poster supports the implementation of current guidelines that recommend the use of cognitive behavioral therapy to treat insomnia in older adults.Item Decreasing 30-Day Hospital Readmissions for Patients with CHF using Teach-Back Method(2022) Callahan, Robin BSN, CVRN-BC; Dr. Cheryl Brohard, PhD RN CNS-ONC AOCN® CHCPA®Background: Congestive heart failure (CHF) affects 6.2 million Americans. CHF has a high readmission rate within 30 days of hospitalization. Using the teach-back method (TBM) for discharge is an effective means of helping patients with self-management at home. PICOT: In hospitalized adult patients with CHF, how does TBM for discharge teaching compared to routine discharge teaching affect hospital readmission rates of acute exacerbation in 30 days? Synthesis: The evidence demonstrated the TBM increased understanding and compliance which reduced hospital readmission rates. Plan for Change: Implementation will occur in an acute care facility with patients with CHF, where RNs will be trained on TBM and accompanied by written education materials for the patient. The plan will be evaluated with a phone call 48-72 hours post discharge to assess patient’s knowledge.Item Decreasing Congestive Heart Failure Readmission with Telemonitoring(2018-04-13) Ezebuiro, MaryDirect congestive heart failure costs are expected to rise from 21 billion dollars in 2012 to an estimated 53 billion dollars in 2030. There is a 20% lifetime risk of developing congestive heart failure with a five-year mortality rate higher than many cancers (McTernan, et al., 2016). This poster provides information about using CHF app, EBP guidelines, and multidisciplinary care to reduce mortality and readmission rates, and improve quality of life.Item Developing Critical Thinking through the Use of Unfolding Case Studies(2022) Vickers, Karen BSN, RN; Dr. Cheryl Brohard, PhD, RN, CNS-ONC, AOCN®, CHPCA®Background: Studies indicate that when nursing students transition to clinical practice, they do not possess enough critical thinking skills to deliver safe patient care. Unfolding case studies provides an opportunity for students to analyze, predict, interpret, and transform knowledge. PICOT: In baccalaureate nursing students, how does unfolding case studies compared with traditional lectures affect critical thinking over a semester? Synthesis: Eight articles met the inclusion criteria with 1-Level II, 6- Level III, and 1-Level IV. Plan for Change: Using the Advancing Research and Clinical Practice through Close Collaboration in Education framework, two unfolding case studies will be implemented in the first three semesters. During the capstone course, students will be given a pre-/post-test Health Science Reasoning Test as a means to evaluate the students’ critical thinking. Unfolding case studies is an effective strategy in improving students' critical thinking and connecting theory to clinical practice.Item Dietary Fiber to Improve Metabolic Syndrome in Adults(2018-04) Butella, HilaryThis evidence based project explores evidence supporting the effectiveness of psyllium fiber dietary supplementation to improve hypertension, hyperlipidemia, glycemic control, weight and waist circumference in adult patients meeting criteria for metabolic syndrome diagnosis. The project recommends that psyllium fiber dietary supplementation be included in clinician recommended therapeutic lifestyle changes for this patient population.Item Early Identification of Maternal Depression Symptoms by Instituting a Postpartum Depression Screening(2018-04-13) Rosser, ChristinaThis evidence-based practice project explores the necessity and feasibility of a practice change of administration of the Edinburg Postnatal Depression Scale (EPDS) by pediatric providers to mothers at every well child appointment, up to one year of age, increasing the rates of capture and referral of patients with postpartum depression.Item The Effect of High-Fidelity Simulation on the Graduate Nurse’s Anxiety(2022) Lewis, Jazmine BSN, RNC-MNN; Dr. Cheryl Brohard, PhD, RN, CNS-ONC, AOCN®, CHPCA®Background: When a student has recently graduated from nursing school and passed their licensure exam, they are called a graduate nurse. The use of high-fidelity simulation can help the nursing students prepare for their professional roles. PICOT: In graduate nurses how does nursing education with high-fidelity simulation compared with nursing education with only low-fidelity simulation affect graduate nurses’ anxiety within the first year after graduation? Synthesis: Using CINAHL, PubMed, and INACSL literature search engines, only 7 articles were found, 3-Level I, 1-Level II, 3-Level III. Plan for Change: Using the ARCC-E model to guide change, high fidelity simulations are recommended for graduate nurses’ education to prepare them for the transition into the work setting and decrease anxiety.Item Experiential Learning to Teach Cultural Competency in Nursing(2020-05-01) Schulte, AnitaCultural competence of healthcare professionals, especially nurses, has been identified as a key factor in reducing health disparities. However, despite the widespread recognition of the importance of cultural competence, curricular strategies to teach this skill have yielded mixed results. The purpose of this evidence-based practice project is to implement experiential learning strategies such as simulation, cross-cultural immersion, and service-learning to improve cultural competence in undergraduate nursing students.Item Home Health Transitional Care Interventions (TCI) to Reduce Rehospitalization in Geriatric Patients with Heart Failure(2019-04-12) Munuve'-Akinnola, RoseThis evidence-based project evaluates the evidence that high-intensity transitional care interventions at home are effective at preventing re-hospitalization, reducing mortality and improving quality of life in geriatric patients with heart failure. The project recommends consistent implementation, at home, of an interdisciplinary team, use of a standardized tool to assess re-hospitalization risk and regular use of the Teach Back method for patient and caregiver education (AHRQ, 2015). These strategies should be part of the standard outpatient intervention.Item Improving Pregnancy Outcomes by Implementing Centering Pregnancy into Prenatal Care(2020-05-01) Elegon, IyoboThis evidence-based research project discusses the benefits of group prenatal education and its outcome on the rates of birth rates and preterm births. With the limited amount of time given in clinical practice, there becomes an inability to educate pregnant patients with enough prenatal education. There is substantial evidence that reveals a lack of prenatal education is linked to an increase risk of preterm birth and low birth rates. This project recommends that Centering Pregnancy, which is a group prenatal education program, decreases those risks as compared to standard prenatal education. The tools used in the Centering Pregnancy prenatal education program not only benefits the mother by educating her with important knowledge but decreases the risks of premature birth and low birth rates in their unborn child.Item Improving Quality of Life with Early Initiation of Palliative Care in Patients with Advanced Cancer(2020-05-01) Gnan, CaitlinThis evidence-based project explores the effect of initiating palliative care early in advanced cancer to improve patients’ quality of life, anxiety, and depression. Medical oncology providers will identify patients with distant metastases, late-stage, or life-limiting cancer, educate identified patients on benefits of palliative care, and refer them for initial palliative care consultation. Quality of life, depression and anxiety will be measured for each patient prior to consultation and after three months of receiving dedicated palliative care services, using the MQOL, PHQ9 and GAD7 questionnaires.Item Increasing the Diagnosis & Management of Generalized Anxiety Disorder in Primary Care by Utilizing the GAD-7(2020-05-01) Talton, RobinThis evidence-based project (EBP) implements the GAD-7 as a screening tool in diagnosing and treatment management of patients with GAD. The evaluation of the screening tool includes comparing the number of patients diagnosed with GAD before and after implementing the GAD-7 into practice, and HCPs perspectives of the standardized tool in diagnosing and treatment management of patients.Item Increasing the HPV Vaccination Rates: A Primary Care Initiative(2019-04-12) Olvera, OznyAccording to the CDC, about 1 in 4 people currently have the HPV virus (2018). The current CDC recommendation is that all children ages 11-12 years of age receive two HPV vaccines 6-12 months apart. As infection with HPV progresses, it can lead to preventable cancers such as cervical cancers in women and penile cancers in men. Despite the availability of the HPV vaccine, vaccination rates continue to be low. With the susceptibility of HPV preventable cancers, the lowered vaccination rates are a cause for concern to healthcare providers. Research has shown that when a provider makes a strong recommendation for the HPV vaccine, parents are more likely to vaccinate. This evidence-based practice project explores the effects of a presumptive provider-led recommendation to vaccinate versus ancillary staff on compliance in vaccination.Item Ketogenic Diet To Improve Outcomes In Patients with Type 2 Diabetes Mellitus(2018-04-13) Yap, MarkThe recommendation of a ketogenic diet for type 2 diabetes is often overlooked in favor of the traditional high carbohydrate, low-fat diet due to lack of knowledge. There is accumulating evidence to show that a ketogenic diet can have a significant effect on treatment. This evidence-based project compares a ketogenic diet to high carbohydrate, low-fat diet and shows that in comparison, ketogenic diets show improved glucose control, A1C levels, a good safety profile, and warrant inclusion in diabetic therapy.Item Nurse Champions to Reduce Catheter-Associated Urinary Tract Infections(2020-05-01) Dunmore, AlexisThis evidence-based project aims to address the implementation of a unit nurse champion compared to having no unit-specific nurse educator in efforts to decrease the occurrences of CAUTIs on the inpatient hospital unit over a three-month timespan. The nurse champion can be identified as a staff nurse who cares for patients alongside fellow bedside nurses while serving as an expert educator of evidence-based practices (Mount & Anderson, 2015). The nurse champions will receive training from evidence-based project (EBP) mentors, and; the newly trained scholars (nurse champions), transfer of peer-to-peer knowledge, and assiduous administration buy-in, will foster the implementation of best evidence and quality care leading to a decrease in CAUTIs (English, 2016).Item PDMP Utilization to Decrease Prescription Opioid Overdose Deaths(2019-04-12) Ugochukwu, KenechukwuOn average, 130 Americans die every day from an opioid overdose. From 1999 to 2016, more than 200,000 people died in the U.S. from overdoses related to prescription opioids (CDC, 2017). Prescription Drug Monitoring Program (PDMP) is an electronic database that tracks controlled substance prescriptions in a state. This EBP project focuses of the utilization of Prescription Drugs Monitoring Programs (PDMP) to decrease overdose deaths related to prescription Opioid abuse.Item Pressure Injury Prevention in Critically Ill Hospitalized Adult Patients(2022) Masenda, Mercy BSN, RN, CCRN, CVRN; Dr. Cheryl Brohard, PhD, RN, CNS-ONC, AOCN®, CHPCA®Background: Hospital-acquired conditions (HAC’s) such as pressure injuries are prevalent on a global scale. Over 2.5 million people develop pressure injuries with a cost of up to $50,000 each year in the U.S. Pressure injuries are painful to the patients, prolong hospital stays, and cause more than 60,000 death each year. PICOT: In critically ill, hospitalized patients, what is the effect of the continuous lateral rotation therapy (CLRT) compared to manual repositioning every two hours in preventing pressure injuries over a three-month period. Synthesis: Studies showed that not only does CLRT work, but that it is superior to manual repositioning in preventing pressure injuries. The purpose of this evidence-based project is to implement continuous lateral rotation therapy (CLRT) in place of manual repositioning, the gold standard in pressure injury prevention.Item Prioritizing Acute Care Education in Renal Transplant Recipients – An Evidenced Based Approach with the ACE STAR Model of Knowledge Transformation(2020-05-01) Garcia, FerdinandThis EBP project focuses on prioritizing acute care education with an EBP approach with the ACE STAR Model of Knowledge of Transformation. It demonstrates the positive outcomes of acute intervention have improving medication adherence rates, decreasing readmission rates, the average length of stay, enhancing patient educational experience and reducing costs to hospitals.Item Probiotics Treat and Prevent Antibiotic-Associated Diarrhea(2019-04-12) Pongvachararak, AaronThis evidence-based project explores evidence supporting the effectiveness of probiotics to treat and prevent antibiotic-associated diarrhea in adult patients ages 18-65 years. The project recommends that probiotics be prescribed concurrently with clinician recommended antibiotic treatment for this patient population.
