Abstract

Diabetic patients in underserved communities can benefit from establishing a relationship with a PCP to manage diabetes and prevent avoidable emergency room visits. Collaborative care programs can be used to increase access to primary care providers in patients with diabetes who live in underserved communities (Carter et al., 2016; Chwastiak et al., 2017; Karter et al., 2017; Soto Mas et al., 2019). Emergency room nurses can collaborate with hospital administrators and community leaders to provide access to resources such as education on diabetes management.
